How do I go about setting up the plan?

  1. Complete the online Application Form
  2. Complete an enrolment form for each plan member
  3. Send us a cheque for $250 made payable to “Winflex Health Solutions Inc.”
  4. Electronically sign the Trust document (which will be emailed to the contact person on the application form).
